Interventional pain management uses precise, image-guided procedures to find and treat the source of spine, joint and nerve pain, often when time, medication or physical therapy have not been enough. At Motionis Medicine in Downtown Brooklyn, Dr. Tanuj Palvia, MD is board certified in interventional pain medicine and anesthesiology and evaluates each patient personally.

What interventional pain management means

An interventional pain physician combines a careful diagnosis with targeted procedures. Instead of treating pain only with medication, the goal is to identify the structure that is generating the pain, such as a spinal nerve, a facet joint, the sacroiliac joint or a tight muscle, and then treat that structure directly when appropriate.

Procedures are performed under fluoroscopic (live X-ray) or ultrasound guidance for precision and safety, and are chosen as part of a broader care plan, not on their own.

Who may benefit from a pain specialist evaluation

You may benefit from a specialist evaluation if:

  • Pain has lasted longer than expected
  • Physical therapy or medications have not provided enough relief
  • Imaging shows wear and tear, but you would like to avoid surgery
  • Pain interferes with work, sleep or daily activity
  • Symptoms are recurring or getting worse

Image-guided treatments offered

Regenerative options, including PRP therapy, are described under non-surgical orthopedics.

Frequently asked questions

What is the difference between a pain doctor and an interventional pain doctor?

A pain doctor may manage pain mainly with medication and therapy. An interventional pain physician also performs image-guided procedures, such as injections and nerve treatments, to diagnose and treat the source of pain.

Are interventional procedures a first step?

Not always. Many plans begin with conservative care. Procedures are considered when they fit your diagnosis and goals, and are discussed with you first.

Is surgery the only option if injections do not work?

No. Non-surgical and minimally invasive options are explored thoroughly, and Dr. Palvia helps patients understand whether surgery is truly necessary and what alternatives may be appropriate.
